Maria Machado Memorial Education Program Award

Purpose
This award is presented in honour of Maria Machado, an outstanding pharmacist who was tragically killed in a motor vehicle accident on February 23, 1993. In remembrance, St. Paul’s Hospital Pharmacy Department and the B.C. Branch Council of C.S.H.P. established the award.

Award Description
This award will be given to a B.C. hospital pharmacy department to expand or initiate a patient counseling and/or education program.

Eligibility
All hospital pharmacy departments in British Columbia are eligible for the award provided they have staff pharmacists who are active members of the B.C. Branch C.S.H.P. Deadline for submission is August 31st.

Selection Criteria
The B.C. Branch C.S.H.P. Awards Committee will review each submission based on the following criteria:

  • Patient counseling and/or education program (new or expanded)
  • Patient information handouts/pamphlets that are clear, concise and innovative.
  • Active member of B.C. Branch C.S.H.P.
  • Letter of approval from Director of Pharmacy.

Award Presentation
The award will consist of a certificate and a cash award of $250.00. The B.C. Branch C.S.H.P. Awards Committee will reassess this award on a yearly basis.

Awards
