PURPOSE
The purpose of this award is to recognize outstanding attendance at B.C. Branch
C.S.H.P. sponsored events.
AWARD
DESCRIPTION
This award is presented to three hospitals (large, medium and small) based the size of their pharmacy staff as stated below. Note that the FTEs count includes all pharmacists including directors, supervisors, and permanent full time and part time pharmacists.
- The large hospital award is presented to a hospital with twenty or more full time equivalent (FTE) pharmacists.
- The medium hospital award is presented to a department with ten to 19.99 FTEs
- The small hospital award is presented to a department with less than ten FTEs.
ELIGIBILITY/APPLICATION
DEADLINE
All hospital pharmacy departments in British Columbia are eligible for this award provided they have staff who are active members of C.S.H.P.
SELECTION
CRITERIA
The B.C. Branch C.S.H.P. Programs Chair will review the attendance record for
each B.C. Branch C.S.H.P. sponsored events during one “B.C. Branch year”
(including the AGM of the previous year up to but not including the AGM of the
current year).The selection will
be made by the Awards Committee Chair and is based on the greatest percentage of
pharmacists attending these events during the year.
AWARD
PRESENTATION
The award will consist of a plaque engraved acknowledging the current
recipients. The plaque will be presented at the AGM. Plaques will be forwarded
from past winners to current winners annually. The B.C. Branch C.S.H.P. Awards
Committee will reassess this award on a yearly basis.
